The 4.2rev04 update was a batch IOS update released shortly before 4.3, much like 3.4rev02.

Fixed HackMii Installer Userland+Syscall IOS exploit in all IOSes (including IOS58, which was previously not on NUS)
ES_AddTitleFinish now checks signatures
IOS9 now has a build tag
Removed the Internet Channel patch from IOS15 (by removing the NAND Boot Program), probably by accident
- IOS21 still has this patch
Several IOSes have been replaced with copies of other IOSes
- IOS31, IOS33, and IOS34 are now identical
  - Note that IOS35 and IOS36 were already identical to IOS31
- IOS53 has been replaced with a copy of IOS55
Added IOS80 to prepare for 4.3.
