Title contents are stored in the subfolders of /title. Title A-B is stored in /title/A/B, for example, IOS9 is in /title/00000001/00000009.

A quick overview about the folders:

/00000001 - System Titles (System Menu and IOS)

/00010000 - Disc-based games (holds save files)

/00010001 - Installed Titles

/00010004 - Game Channels (games that use a channel - Mario Kart Channel and Wii Fit (Plus) Channel)

/00010005 - Game DLC

/00010008 - Hidden Channels (EULA, Region Select, Set Personal Data)
